A pharmacy benefit manager is turning the grocery cart into a clinical tool. True Rx Health Strategists, the Indiana-based, pharmacist-led PBM, has partnered with Instacart to embed the platform's Health Fresh Funds stipend program directly into employer pharmacy benefits — a first for the PBM sector. Starting January 2027, employer clients whose members are enrolled in True Rx's True Outcomes: Weight Management or True Outcomes: Diabetes Management programs will be able to fund monthly grocery stipends redeemable across Instacart's network of more than 2,200 retail banners.
Why It Matters
The move lands at a pressure point for self-funded employers. GLP-1 demand has strained pharmacy budgets, yet FDA labeling for those medications explicitly calls for use alongside reduced-calorie diets and increased physical activity — meaning a prescription alone is rarely sufficient. True Rx has responded by opening its weight management program to any employee who wants to lose weight, regardless of whether their plan covers GLP-1s. The Instacart Fresh Funds integration gives that program a tangible grocery component: employers select a monthly amount of $50 or $100 per member, the balance refreshes each month, and funds are active for up to six months as long as the member stays enrolled.
Because access to Fresh Funds requires completing a qualifying session with a True Rx dietitian or clinician, employer dollars flow only to actively engaged members. Dietitians build personalized meal plans, share recipes, and then use members' actual Fresh Funds purchase data to guide follow-up conversations — effectively closing the loop between a nutrition plan and what ends up in the refrigerator. Aggregate utilization and clinical outcomes are tracked and reported back to employers. True Rx bills only for funds members actually spend, consistent with the company's transparent-pricing model.
Pilot Results
A six-month pilot of True Rx's weight management program — focused on members using GLP-1 medications — showed participants achieving weight loss of up to 15%, with 90% completing monthly virtual visits. That engagement rate matters in the broader context of food-as-medicine strategies, which have gained significant traction across restaurant and foodservice industry discussions around nutrition as operators and suppliers increasingly field questions about medically tailored meal programs.
"Medication plays an essential role, and so does nutrition," said Adrienne Williams LaBorwit, CEO of True Rx Health Strategists. "By connecting clinical support with Instacart Health Fresh Funds, we're giving employers a practical way to build food as medicine into the pharmacy benefit."
Sarah Mastrorocco, vice president and general manager of Health at Instacart, described the partnership as a scalable way to translate nutrition guidance into actual purchasing behavior: "Through Fresh Funds and our network of trusted grocers, we're helping True Rx members use employer-funded dollars to shop for nutritious foods that support their health goals, with the convenience of delivery to their door."
Broader Food-as-Medicine Trend
The partnership reflects a widening intersection between grocery delivery infrastructure and clinical health programs — a trend that has direct implications for grocery retail and food retail operators watching how employer-funded food benefits reshape where and how workers buy fresh produce, proteins, and dairy. Instacart's reach across nearly 100,000 store locations means the stipend can be redeemed at a wide range of regional and national grocers, not a closed network of specialty retailers.
True Rx unveiled the partnership during a webinar featuring Blue Zones founder Dan Buettner, signaling that the company is positioning food-as-medicine not as a peripheral wellness perk but as a core component of pharmacy benefit strategy.
